Common Use

Arpamyl is a Ca2+-channel blocker that relaxes veins and arteries, making it easier for the heart to pump and reducing its workload. Calcium triggers smooth muscle contraction, and by blocking its entry into myocytes, Arpamyl relaxes arterial muscles, improves oxygen delivery to the heart, and reduces total peripheral resistance, systemic blood pressure, and afterload. It also slows the conduction of electrical impulses in the heart, decreasing the rate of contraction. Arpamyl is commonly used to treat hypertension, angina, and certain irregular heartbeats.

Dosage and Direction

Take Arpamyl orally with or without food. The daily dose ranges from 180-480 mg, which can be taken as a single dose or divided into two or three intakes. Always follow your doctor's prescription and never exceed the recommended dose.

Precautions

• Avoid alcohol, grapefruit juice, and grapefruit while taking Arpamyl, as they can worsen side effects or interact with the drug.
• Do not rise too quickly from a sitting or lying position to avoid dizziness.

Contraindications

• Arpamyl is not recommended for patients with hypersensitivity to its components, severe left ventricular dysfunction, hypotension (systolic pressure less than 90 mm Hg), or cardiogenic shock.
• It is also contraindicated in patients with sick sinus syndrome, atrioventricular block degree II-III, atrial flutter, atrial fibrillation, or accessory bypass tract syndromes (e.g., Wolff-Parkinson-White, Lown-Ganong-Levine) unless a functioning artificial ventricular pacemaker is present.

Side Effects

Arpamyl is generally well tolerated, but common side effects include dizziness, weakness, fainting, swelling of the lower extremities, rash, headache, and constipation. Severe side effects like jaundice, difficulty breathing, or rashes require immediate medical attention.

Drug Interactions

• Arpamyl inhibits alcohol elimination and increases the efficacy of doxorubicin.
• Certain cytotoxic drug regimens (e.g., COPP, VAC) may decrease its absorption.
• Co-administration with aspirin may increase bleeding risk, and with digoxin, it can elevate serum digoxin levels, leading to toxicity.
• Additive effects may occur when combined with oral antihypertensive agents.
• Inform your doctor about medications like Disopyramide, Flecainide, Quinidine, Phenobarbital, Cyclosporin, or lithium-containing drugs, as Arpamyl may interact with them.

Missed Dose

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and return to your regular schedule.

Overdose

Overdose symptoms may include a slow heartbeat and fainting. Seek immediate medical attention if you suspect an overdose.

Storage

Store Arpamyl at room temperature (15-30°C or 59-86°F) away from light, moisture, pets, and children.
