Common use

Rifampin is an antibiotic used primarily to treat tuberculosis (TB) and eliminate meningococcal bacteria from the nose and throat. It works by inhibiting bacterial RNA synthesis, stopping bacteria from multiplying. Rifampin is often used in combination with other antibiotics to prevent drug resistance. It is also sometimes prescribed for off-label infections, such as prosthetic joint infections.

Dosage and direction

The usual adult dose for TB is 600 mg once daily, taken on an empty stomach, typically 1 hour before or 2 hours after meals. Dosage may vary based on the specific infection and patient weight.

Precautions

• Can cause liver damage, especially with hepatotoxic drugs
• Regular liver function tests recommended
• May cause harmless red-orange bodily fluid discoloration

• Severe liver disease without monitoring
• Significant interactions with HIV medications and antiepileptics
• Not recommended as single-agent therapy

Possible side effects

• Serious: Hepatitis, allergic reactions, blood disorders
• Seek medical help for jaundice or unusual bleeding

Drug interactions

Rifampin is a strong enzyme inducer that affects many medications:

• Reduces effectiveness of oral contraceptives
• Impacts warfarin, antiretrovirals, and anticonvulsants
• Requires dose adjustments of other medications
• Avoid alcohol to prevent liver toxicity

Missed dose

• Take as soon as you remember
• If near next dose time, skip the missed dose
• Never double doses

Overdose

Symptoms include:

• Nausea and vomiting
• Confusion or seizures
• Orange-red discoloration
• Potential liver failure

Seek immediate medical attention for overdose.
