Isordil is prescribed for the treatment and prevention of angina caused by coronary artery disease. Angina occurs when the heart muscle does not receive enough blood. This medication is often used before physical activities such as exercise or sexual activity. Isordil belongs to the nitrate class of drugs, which helps relax and widen blood vessels, allowing blood to flow more easily to the heart.
The usual initial dose of Isordil is 5 mg to 20 mg, taken two or three times daily. For maintenance therapy, doses between 10 mg and 40 mg, two or three times per day, are recommended. Some patients may require higher doses, depending on their condition and therapy. To minimize tolerance, a daily dose-free interval of at least 14 hours is necessary. Sit down before taking Isordil. It should be taken by mouth with a glass of water, dissolved under the tongue, and not crushed or chewed. Take this medication on an empty stomach, at least 30 minutes before or 2 hours after meals. Use it regularly, but do not exceed the prescribed dose or duration. Do not stop taking Isordil suddenly, as this may worsen symptoms. Always consult your doctor or healthcare professional.
Isordil may cause severe hypotension, especially when standing upright. Patients with hypertension or those who are volume-depleted should use this medication cautiously. Hypotension from Isordil may be accompanied by paradoxical bradycardia or increased angina pectoris.
Inform your doctor if you have a history of heart attack or heart failure, an allergy to isosorbide dinitrate, nitrates, other medications, foods, dyes, or preservatives, or if you are pregnant, trying to conceive, or breastfeeding.
Isordil is contraindicated in patients with an allergy to isosorbide dinitrate or any of its ingredients. Although allergic reactions to organic nitrates are rare, they can occur.
The most common side effect of Isordil is a severe headache, especially at high doses. Other side effects may include lightheadedness, hypotension, syncope, crescendo angina, and rebound hypertension. Hypotension may be severe enough to require discontinuation of therapy. Isordil can also cause dizziness, palpitations, and weakness due to a drop in blood pressure.
Report the following side effects to your doctor immediately: bluish discoloration of lips, fingernails, or palms; irregular heartbeat; palpitations; low blood pressure; nausea; vomiting; persistent headache; or unusual weakness or tiredness. Common side effects like flushing of the face or neck or rash usually do not require medical attention but should be reported if they persist or become bothersome.
Taking Isordil with sildenafil (Viagra), tadalafil (Cialis), or vardenafil (Levitra) can increase blood pressure-lowering effects and may cause excessive blood pressure reduction. Avoid using Isordil with medications for erectile dysfunction, high blood pressure, or other treatments for angina or heart failure.
If you miss a dose, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and take only the next scheduled dose. Do not take double or extra doses.
In case of overdose, contact a poison control center or emergency room immediately.
Store Isordil at room temperature, approximately 25°C (77°F). Protect it from moisture and light by keeping it in a light-resistant, tightly sealed container. Keep it out of reach of children. Dispose of any unused medication after the expiration date.
