Common Use

Alfacip, an analogue of vitamin D, contains the active ingredient alfacalcidol. It is used to treat conditions related to impaired calcium metabolism caused by 1-α hydroxylation disorders, particularly in cases of reduced renal function. Conditions treated by Alfacip include:

  • Renal osteodystrophy
  • Hyperparathyroidism (parathyroid hormone deficiency affecting calcium and phosphorus absorption)
  • Hypoparathyroidism
  • Neonatal hypocalcemia
  • Nutritional and malabsorptive rickets and osteomalacia
  • Pseudo-deficiency (vitamin D-dependent) rickets and osteomalacia
  • Hypophosphatemic vitamin D-resistant rickets and osteomalacia
  • Osteoporosis

Dosage and Direction

The standard daily dose of Alfacip is 1 mcg/day. Dosage adjustments may be necessary for:

  • Elderly patients
  • Neonates and premature infants
  • Children weighing under 20 kg

Precautions

Patients taking Alfacip should undergo regular monitoring to prevent hypercalcemia, including:

  • Plasma calcium levels
  • Parathyroid hormone levels
  • Alkaline phosphatase tests
  • Radiographic and histological examinations

Patients with chronic renal failure require close monitoring, as prolonged hypercalcemia may worsen kidney function.

Contraindications

Alfacip should not be used by patients with:

  • Hypercalcemia
  • Metastatic calcification
  • Hyperphosphatemia (unless occurring with hypoparathyroidism)
  • Hypermagnesemia
  • Vitamin D toxicity
  • Hypersensitivity to vitamin D or its analogues

Possible Side Effects

Adverse reactions may include:

  • Hypercalcemia
  • Malaise, tiredness, weakness
  • Nausea and vomiting
  • Headache and dizziness
  • Constipation or diarrhea
  • Dry mouth
  • Abdominal or gastrointestinal pain (e.g., heartburn)
  • Heart palpitations
  • Bone, muscle, or joint pain

Drug Interactions

Long-term use of anticonvulsants (such as phenytoin) may alter vitamin D and calcium metabolism, leading to metabolic bone disease, hypocalcemia, or seizures. Calcium levels should be monitored in patients on antiepileptic drugs.

Missed Dose

If a dose is missed, take it as soon as rememberer, unless it is nearly time for the next dose. Do not double the dose to compensate. Maintaining a consistent daily schedule helps prevent missed doses.

Overdose

Symptoms of overdose may include:

  • High fever
  • Elevated blood pressure
  • Irregular heartbeat
  • Severe stomach pain

Storage

Store at room temperature (59-77°F / 15-25°C) away from light, moisture, children, and pets. Do not use after the expiration date.
